If you are the first person registering an organization please upload the following attachments. If you do not have these attachments available at the time of registration you can add them later, once your account is set up, under “My Profile.” Please review documents on an annual basis and update as necessary. Download the City of Elk Grove’s W-9 form, enter all of the required information and upload a signed copy.
